Application for Printing Certificate of Graduation Document That Is Lost/Damaged

If your graduation document is lost or damaged, you may apply for a Certificate of Graduation Document as a replacement.
Application Requirements
1. Fill Out the Certificate Printing Form

Download the form via the following link: https://bit.ly/formulir-cetak-surket-dokumen-kelulusan.

The form can be filled out directly online — no need to print.
2. Attach Supporting Documents
If the Document Is Lost:

Attach a Loss Certificate from the police that clearly states the type of lost document.
Examples:

If the lost document is a Diploma, the police certificate must state “Diploma.”

If the lost documents are Diploma and Transcript, the police certificate must state “Diploma and Transcript.”
If the Document Is Damaged:

Attach a photo of the damaged graduation document as supporting evidence.
Application Process

Send the completed form and supporting documents via email to: halobinus@binus.edu.

Your application will be reviewed and processed within a maximum of 3 working days for approval.

The printing fee is Rp 200,000 per document.

Payment instructions will be provided via email once your request is approved.

Important Notes

The certificate printing process will be carried out within 3 working days after payment is completed.

The Certificate issued by BINUS is not identical to the original graduation document, but an official letter with the university letterhead.
